Are you or a loved one about to leave the hospital or a care facility after a long stay? The transition process can be difficult, but we can make it easier for you! We provide support for individuals and their families during this period, including assistance before and after discharge, together with our service that focuses on developing independent living skills, supporting diverse interests, and providing additional help as needed.

  • Supporting Client Interest
    We are proud to support and encourage our client’s hobbies and interests, prioritizing their safety and well-being. Our caregivers are equipped to foster these interests, whether it be through activities such as knitting, walks, music listening, playing an instrument, or even going to movies.
  • Independent Living Skilled Training
    Our focus is to aid our clients in acquiring fundamental life skills in their own homes that can lead to greater independence. These skills may include cooking, making beds, light cleaning and dusting, and other simple tasks that can be safely done without assistance but can nevertheless motivate clients to become more self-sufficient.
  • Extended Transition Service
    Our assistance can be extended two months prior to discharge and up to 12 months after discharge from an institution/facility to ensure that patients receive the best possible treatment and achieve the highest level of recovery, to reduce the likelihood of re-hospitalizations.
