POSITION: Transition Coordinator

Job Summary:

Transition coordinators specialize in helping patients with disabilities or mental health disorders obtain the services necessary to transition from assisted living programs into more independent living arrangements.

Qualifications/Educational Requirements:

  • 1.Graduate of an accredited High School or GED.
  • 2.Ability read, write, and follow directions.
  • 3.At least 18 years of age and has demonstrated competency.
  • 4.Trained in certified first aid.
  • 5.A personal care worker shall have completed agency training or have verified experience in the provision of home care tasks to consumers and passed a competency evaluation.
  • 6.Work positively and favorably with clients, families, and staff.
  • 7.Demonstrate compassion, responsibility, and cheerful attitude
  • 8.Able to demonstrate competency in all areas of training for personal care.

Responsibilities/essential functions:

The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

  • 1.The Transition Coordinator is the lead for ensuring that any individual who wishes to move to a more integrated setting from the adult care home or state psychiatric hospital is provided with clinically indicated and appropriate behavioral health services and supports.
  • 2.The services provided prepare individuals for discharge and assist during adjustment period immediately following discharge from an institution. This is a mobile position with work done in a variety of locations. The Transition Coordinator will work with consumers in the communities.
  • 3.Provide assistance to clients regarding their personal situation and health issues. The Transition Coordinator will help with physical activities. Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists and fingers.
  • 4.A high level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance. This will require exceptional interpersonal skills, highly effective communication abilities and the propensity to make prompt independent decisions based upon relevant facts. Problem solving, negotiation, arbitration and conflict resolution skills are essential to balance the needs of both internal and external customers.
  • 5.Follows the instructions of the Supervisor in providing personal assistance services
  • 6.Have knowledge of agency policy and procedures.
  • 7.Observe and report any safety hazards found in the client’s home or any significant observations regarding the client.
  • 8.Be trained in first aid.
  • 9.Maintain client confidentiality/adheres to HIPAA requirements and agency policy and procedures manual.
  • 10.Possess the ability to follow written and oral instructions.
  • 11.Report abnormal findings in client’s conditions as observed and per service plan to the Supervisor.

Environmental Elements

Employee works in an office environment with moderate noise levels, controlled temperature conditions and no direct exposure to hazardous physical substances. Employees may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures.

Employee will have to travel to a variety of client homes and perform in conditions that vary greatly depending upon the client’s home environment. Some homes will be clean, neat, and maintained at a comfortable temperature. Other homes may be cluttered, dirty, with an uncomfortable temperature.
